I understand that I am aware of my own health and physical condition, and have knowledge that my participation in any exercise program may be injurious to my health, I am voluntarily participating in a physical activity. Having such knowledge, I hereby acknowledge this release, any representatives, agents, and successors from liability for accidental injury or illness which I may incur as a result of participating in said physical activity. I hereby assume all risks connected therewith and consent my participation in said program. I agree to disclose any physical limitations, disabilities, ailments, or impairments which may affect my ability to participate in any Hype Fitness program.

We love to share photos and videos on our social media pages- and have partnered with House of Gemini Productions as our media liaison, however this is completely optional and we respect your choice and privacy. Please check the box that applies to you, we will be sure to omit you from media if you choose not to participate and you are always free to let us know if you change your mind either way.

I understand that if I am a CLASS PASS holder, enrollment is on a class-by-class basis. Class Pass enrollment must be verified prior to EACH class to ensure that class spots have not been filled for the session. Please ensure your attendance is anticipated by pre-registering each week.
